Nattokinase — timing, dose & interactions

Enzyme from natto (fermented soybeans) that breaks down fibrin, the protein in blood clots. In a meta-analysis of six randomized trials it lowered blood pressure by about 3/2 mmHg, while slightly raising cholesterol and blood sugar. Because it acts on clotting, it adds bleeding risk to blood thinners.

Timing

When to take it

Often taken away from meals, once or twice daily. Dosing is by enzyme activity (fibrinolytic units, FU), not just milligrams.

Interactions

Nattokinase with other supplements

Safety

Medications & health conditions

CautionWarfarin

Nattokinase adds to bleeding risk. It breaks down fibrin, and a brain bleed was reported in a patient taking it with aspirin. Don't combine with blood thinners without your prescriber.

Human data
CautionSurgery in the next 2 weeks

Stop nattokinase before surgery. It affects clotting — tell your surgeon; it's commonly paused 2 weeks before.

Mechanism-based
NoteBlood pressure medication

Nattokinase may lower blood pressure slightly. In a meta-analysis it lowered blood pressure by about 3/2 mmHg — small, but it adds to your medication.

Strong evidence
CautionPregnant or breastfeeding

Nattokinase: no safety data in pregnancy. Nattokinase has not been studied for safety in pregnancy or breastfeeding. Avoid unless your clinician recommends it.

Buying guide

What to look for on the label

  • Nattokinase

    • Labeled in fibrinolytic units — 2,000 FU (about 100 mg) is common
    • Vitamin K2–removed versions if you take warfarin

FAQ

Common questions

When is the best time to take Nattokinase?

On waking / Bedtime. Often taken away from meals, once or twice daily. Dosing is by enzyme activity (fibrinolytic units, FU), not just milligrams.

Should Nattokinase be taken with food?

It can be taken with or without food. Often taken away from meals, once or twice daily. Dosing is by enzyme activity (fibrinolytic units, FU), not just milligrams.

Can you take Nattokinase with Ginkgo?

Be cautious — both can affect clotting, and together the effect may add up. Avoid the combination if you take a blood thinner, bruise easily or have surgery coming up. Otherwise watch for unusual bruising or bleeding.
